How do I complete the enrollment packet on my computer?
Step 1: Using a PDF viewer, open the enrollment packet available in the form above. We like Adobe Acrobat DC, a free program for Windows & Mac systems.*

Step 2: Fill out the enrollment paperwork using the Fill and Sign tool.

Step 3: Save your completed enrollment packet to your computer. Please use a file name that has your child's first and/or last name.

Step 4: Upload the version from Step 3 to the form above and submit!

Why am I unable to upload my enrollment packet and/ or immunizations to the form?
The form above only accepts documents. Files ending in .doc, .docx, .pdf are examples of document file types. The form does not allow uploads of photos (.jpg, .png)
If you filled out a physical copy of either document, please submit it in person OR upload it using a supported file type.
